Complete Consent Form for Required Change in Immunization Record Keeping
Oak Harbor Public Schools is required to manage student immunization records in a new way through the Washington State Department of Health. As we move to this system, families will need to consent to adding their student's unrecorded vaccination records to the Washington State Immunization database by completing and signing this form that was sent via ParentSquare. Please reach out to your school nurse with any questions about this new requirement.
