Referral Forms

  • Complete the form below to refer your child for Highly Capable Services.  This is specifically for students that are new to the Oak Harbor Public Schools for the 24-25 school year.

     

    1st and 2nd Grade Summer Referral Form

    3rd – 12th Grade Summer Referral Form

     

  • Referral window will open October 1-31, 2024

  • All students in 2nd grade are universally screened and considered for HiCap services by completing the Cognitive Abilities Test Screener (CogAt).  Since all 2nd grade students are screened and considered for HiCap services, a referral is not necessary.
